1. Keep showing me service type required error message when I click frame and lens claim submit.
2. I have been on the phone since 11a it's now 2p and Nothing hasn't been done!!! I'm beyond frustrated! And definately changing providers in the new enrollment.I received the email to activate.. I click the link get bounced here and told I have to go back to my email or call because I need to activate my account.
3. The EOB says date of service was date claim submitted, not actual date.Pls fix the issue on claim.

11. I have had numerous conversations with Davis Vision regarding website issues, and advise them not to tout their "brand new app!" until they can rid it of all its "bugs".
