  • On Friday, 9/17/2021, while doing wound rounds with the provider Dr. Brendon Michels several of the wounds were recorded in the wrong patient charts. Being new to the facility, I am unfamiliar with the residents, so while doing rounds with Dr. Michels, I would ask who we were seeing and then open that chart and enter wound information. Come to find at the end of the day, the wounds were recorded in the wrong patient charts. Therefore the wounds either need to be moved to the correct chart or deleted and retook. In addition, wounds that were being reevaluated where placed in as new wounds, artificially increasing the wound ration.
Since there are about 6 residents affected, it would be easiest to do this by phone. 336-679-8863 is the direct line here at Yadkin Nursing Center in Yadkinville, NC (owned by liberty healthcare) and 315-489-2059 is my cell
