Hi, Completed on app, but comes up with error at end: the form could not be created. a form already exists for the specified data

I get the message: The form already exists, when I want to get my QR-code.

I travel on 13th March. I have been able obtain a Spth form and obtain the QR code for myself. However when I try to complete Spth for my wife I am getting an error message stating this form already exsts. I am completing the form correctly. The difference on her form is that she was diagnosed with Covid on 15th February. We have the UK recovery form valid from 26th February. The form also does not seem to be accepting her seat number on the flight. Can you help please.

My form was ok but my form keeping get the same message The form could not be created, Please try again later . That message Is the same for the last 24 hours and we fly on the 12 of March please help

Hello, I have a severe problem. I have an account and want to register. The system always tells me, that there is already a form and I am not able to go on in process. It tells me to erase the form. When I want to erase the form, it tells me, there is no form existing. What can I do - we have to travell to Tenerife on saturday (5.3.)morning 6.00 o´clock..... Please help us as soon as possible. Hello, i have issues with the form of my son and i cant change it. I wanted to change it from Transit to Fastcontrol, apperently i have done something wrong. After trying to change it (actually i could not change anything), it told me that che QR code is now invalid, but i did not get any other QR code. How should i proceed? I got the code QR for the Fast Control but I could not fill in the form for my wife. The reason might be that the name on the air ticket -Cristina Smessow- is not the same as the name on the sanitary pass -Cristina Albu Smessow- Albu is her maiden name. How shall I proceed to get her fast control QR code?

Unable to generate QR Code error message "Form already exists"

When I try to fill in the spth.gob.es FCS paper, it says I already have one. I did fill in the papers, but they were not filled in correctly, because my flight was canceled. Now I cannot fill in another one which means I can´t go to Spain.

QR code cannot be created due to different data in DCC / QR code and personal information. This is because the surname of my wife is Peiffer-Meyer. But "-" is not accepted as a Character when asking for personal details - here in field "surname". Using PeifferMeyer (without "-") works here, but make problems when reading the surname in the Covid-QR-Code (DCC). Good day, I am trying to create an entry document or get the QR code via the SpTH website. I can get to the health questions and select the files for the digital vaccination certificates. If I try to save afterwards, it tells me that the server cannot be reached and that I should try again later. We have to start driving right away to reach the ferry in time. How should I proceed now?
